When uploading a document via the Insurance Deficiency Request email, the Help Form is linked via a button at the bottom of the upload page. If you have projects handled by more than one administrator in CR-Certify, it is also linked at the bottom of the landing screen you see after you log in.

Expand or collapse content What information should I include in the Help Form?

There are three pieces of information that are required in order to submit the Help Form: First Name, Last Name, and Email Address. These key pieces of information will ensure we will know who to reach out to about the contents included within your form submission.

The other fields, though not required, are also great pieces of information to provide so we can fully understand the context behind your form submission. Procedures or set up may be different depending on what project you're working on, and your company name will help us locate any specific details about your particular contract that may be beneficial depending on what you're trying to achieve. Providing your phone number will allow us to pick up the phone and give you a call if that would lead to better clarity than writing out an emailed response. 

Finally, any and all details you can provide in the "How can we help you?" field are incredibly helpful for answering your question and/or solving the issue that you're facing. Things like what error messages you're seeing, how many times you've attempted something, in what context you need clarification, or what brought you to this form in the first place are all great places to start.

This form is sent to our dedicated help desk team for CR-Certify for review. We hold a standard of a two-business-day turn around for our response time to these submissions.
